We've done a bunch of 'em, never canned though. Different hosts want different things, plus you always have to describe the location, etc.
The two most frequent violators are people on Yahoo (geocities, etc) and MSN (msn groups, etc), both of them are quick to yank the offending site offline once you get the letter in the hands of their lawyers.
DMCA requires you hit the following points though:
1) Signature of the owner of the copyright, or an authorized representative. Some will settle for "electronic", most will accept fax, a few will require wet signature be mailed.
2) Description of the work.
3) Description of the location of the violation.
4) Full contact info for yourself.
5) Statement that the use is NOT authorized by the content owner.
6) Statement that everything you stated above is true and accurate and that you are authorized to act on behalf of the content owner if it is not yourself.
