Market conditions over the last couple of years have more than trained me to strain every last penny out of even the nastiest traffic.
I haven't even looked at PK in probably a year and without doing so now I can tell you what my general approach would be.
1) Analyze existing traffic data and income extensively. You cant tune shit without good stats. "In numbers we trust..."
2) Modernize the behind the scenes stuff. See above, you need to be able to easily track and manage ads and everything else.
3) Retain the existing "feel" of the site. Dont fuck with what works and risk all that traffic. Keep the quality and trust, dont "hooper" the traffic. But there's room to do things like selling the best spots or at least making them your own affiliate links.
4) Send a lot of the traffic to my own sites. I'm the only one I know for absolute certain doesn't shave me and my own shit still converts.
5) Create more pages and more ad spots on existing pages, but again keep the quality high and dont over circle jerk it.
6) A site like PK that has user trust screams out to have some "community" type features added.
I have more ideas, I try not to give away too many of them for free on any given day though. Violation of FFN union rules.
