Mike, double anal is just another way to make sex "extreme"... something for the box cover.
Most guys are sufficiently homophobic that they wouldn't want their dicks even near anther guy's duck, let alone sharing the same oriface. It's just not something that happens every day in the real world, which makes it extreme.
Double Anal, gagging blowjobs, etc are all attempted to "go further" than the next movie, so as to be special. I don't think the performers (see Laura Roxx, example) are clearly aware of the risks involved in this type of activity.
